Penile hematoma is more common in adult men as blood supply is greater in adults and blood vessels are bigger. Men get spontaneous erections at night which can also lead to bleeding and hematoma formation. A hematoma occurs in 2-5% of adult circumcisions.A compressive dressing is usually used to minimize the risk of bleeding and hematoma formation after circumcision.Hematoma risk is decreased with ShangRing circumcision. Injection of medications to create an erection during circumcision increases the risk of penile hematomas.Usually, bleeding after circumcision is minor (a few drops) and stops on its own with mild compression. Occasionally bleeding may be persistent and may require re-opening circumcision incision to control bleeding.When a penile hematoma occurs, if it is not infected it may be managed conservatively with observation. For larger hematomas, exploration, and evacuation of hematoma is usually preferable as it speeds up the healing process.Usually, for hematomas greater than 2 to 3 cm in diameter we advise timely evacuation of hematoma as that will lead to faster recovery and resumption of sexual activity.
